DUE TO CHANGES
IN THE PIAA RULES, FREE SPORT PHYSICALS WILL BE OFFERED ONCE A YEAR ONLY. ANY STUDENT PARTICIPATING OR CONSIDERING
PARTICIPATING IN A SCHOOL SPONSORED SPORT AT ANYTIME DURING THE 2008-2009 WHO
WOULD LIKE A FREE SPORT PHYSICAL SHOULD SUBMIT A COMPLETED APPLICATION. THIS IS THE ONLY TIME, FOR THE SCHOOL YEAR
2008-2009 THAT FREE PHYSICALS WILL BE OFFERED AT TJHS.
FREE PHYSICALS WILL BE SCHEDULED ON THE FOLLOWING DATES:

1. TO SCHEDULE A PHYSICAL WITH
THE SCHOOL DOCTOR: You must
submit the proper forms to the nurse before the following
deadline.
(Sport Physical forms will not be sent home. They must be picked up in the TJHS Health Office by the student or parent. ORIGINAL PIAA COLOR-CODED FORMS MUST BE USED. COPIES WILL NOT BE ACCEPTED.)
2. The school doctor will
not add any student to the appointment schedule after this
deadline. If you miss
the deadline, you must schedule the sport physical with your
own doctor. Completed
physicals performed by private physicians are to be
submitted to TJ’s health office by August 8 before noon to
participate in the first day
of fall sports camp.

SCHEDULED ON A FIRST
COME FIRST SERVE BASIS.
¨ INCOMPLETE APPLICATIONS WILL
BE RETURNED TO STUDENT.

CHILD WILL BE GIVEN THE APPOINTMENT SLIP INDICATING THE DATE AND TIME HE/SHE IS
SCHEDULED TO SEE THE DOCTOR IN HOMEROOM THE DAY OF THE APPOINTMENT.
