Students that are participating in a sport
in the 2010-2011 school year may take advantage of the free sport physicals to be
held at Pleasant Hills Middle School on Tuesday, June 15th ,
Wednesday, June 16th and Thursday June 17th.
Students can pick up a PIAA Eligibility form in the health office and
after completing Sections 1, 2 and 3 of the PIAA sport form, return it to
the health office for the nurse to review, before June 1st at
2:30. An appointment letter will be mailed to the student’s home the
week of June 7th. Physicals are by appointment only.
