1. TO SCHEDULE A PHYSICAL WITH
THE SCHOOL DOCTOR: You must
submit the attached forms to the nurse before the following
deadline.
(Sport Physical forms will not be sent home. The student or parent must pick them up in the PHMS Health Office or main office. ORIGINAL PIAA COLOR-CODED FORMS MUST BE USED. COPIES WILL NOT BE ACCEPTED.)
2. The school doctor will
not add any student to the appointment schedule after this
deadline. If you miss
the deadline, you must schedule a physical with your
own doctor.

DOCTOR.
